Upgrade Request Form

Please fill out this form to provide us important information regarding your upgrade needs.  

NOTE: If you are upgrading a single product to a major version ie. 4.6 to 5.0, or 5.0 to 5.1 you will need to upgrade all of your SalesPad products to the same major version.   This is not needed if you’re upgrading to a minor version.  ie. 5.0 to 5.0.1

If you selected Other, please indicate which software you are wanting to upgrade.
If upgrading desktop, we now offer a 64 bit version. Please select 32 or 64 bit.*
These are very important, you can find the version number on the log in screen.

You can find the Desktop System Information window by clicking the blue '?' in the upper right-hand corner of SalesPad Desktop then select 'System Information'. It contains a lot of helpful information that our tech support team uses to ensure a successful upgrade.
List any relative customizations and/or third-party products you have integrated with Cavallo products (ex. Avatax, Accellos, Binary Stream, EDI, etc.).
